  • naam en contactgegevens;
  • adresgegevens;
  • geboortedatum, indien noodzakelijk;
  • afspraak- en administratieve gegevens;
  • intakegegevens en hulpvraag;
  • relevante persoonlijke omstandigheden;
  • gegevens over het counsellingtraject;
  • sessieverslagen, voortgang en evaluaties;
  • relevante correspondentie;
  • factuur- en betalingsgegevens;
  • gegeven toestemmingen;

  • cliëntdossier en sessieverslagen: in beginsel minimaal 20 jaar;
  • facturen en financiële administratie: gedurende de toepasselijke wettelijke fiscale bewaartermijn;

  • afspraakgegevens: niet langer dan noodzakelijk, tenzij deze onderdeel vormen van het cliëntdossier;

4. Confidentiality and professional secrecy

Information discussed during counselling is, in principle, treated as confidential.

As a Registercounsellor ABvC®, I am bound by professional confidentiality and by the professional standards applicable to my practice.

Information relating to counselling will, in principle, not be shared with third parties without your permission unless there is a legal obligation or another legally valid exception.

Where consultation or information exchange with, for example, a general practitioner, referrer or other healthcare professional is appropriate, your prior consent will be obtained where required.

As a professional, I work in accordance with the Dutch Meldcode Huiselijk Geweld en Kindermishandeling (Domestic Violence and Child Abuse Reporting Code). If there are signs or concerns regarding domestic violence or child abuse, I follow the steps of the reporting code and the professional guidelines applicable to my practice. Where necessary, this may involve consultation with or disclosure of information to the appropriate authorities, while observing professional confidentiality and applicable laws and regulations.

5. Client records

I maintain a client record in order to provide careful and professional services. Only information that is relevant and necessary for counselling, continuity, evaluation and professional accountability is included.

Only the personal information and information about the service provided that is necessary for these purposes is included on the invoice. No substantive information from counselling sessions is included on the care invoice.

The client decides whether to submit a care invoice to a health insurer, unless other arrangements have been agreed in advance.
